Share Your Health Records Story

Have you, a family member or friend experienced a medical emergency when you did not have access to important health records? Have you spent time on the phone or in a medical facility trying to get copies of medical records? Do you have a binder at home with all your health records in it?

We would like to learn about your experiences, both positive and negative, with accessing your medical records, your children’s records, or those of a family member you care for.   Were you able to obtain health records when you needed them the most?  What did you have to go through and why?

We are asking you to tell us your story so that we can share it with people and organizations that are developing a plan in central Illinois and the rest of the state to make it possible for healthcare providers to jointly share health records across different computer systems.  Another name for this is Health Information Exchange.  Your experiences will help us plan a system that works and has “real-life” value.
